Request for counselor recommendation for extracurricular/summer program:
Please note that if a school counselor recommendation is needed for a program, it is the student's responsibility to speak to his/her counselor at least 2 weeks PRIOR to the application deadline and the student MUST complete this form. If ample time is not provided to the school counselor, the counselor will not be able to write a comprehensive letter of recommendation. If students do not follow the appropriate protocol, the counselor will NOT have enough time to complete the recommendation.
